What makes an Eye Look Youthful

The eyes are often one of the first areas of the face to show signs of ageing. You may notice that they look tired, heavy, sad or older than they feel. (Even when you are well-rested)

A youthful eye is not simply an eye without wrinkles. It is a result of balance between the upper eyelid, lower eyelid, brow position, skin quality, and the surrounding cheek support.

Blepharoplasty, or eyelid surgery, is a procedure that aims to restore a fresher, more rested appearance by addressing excess skin, puffiness and changes around the eyelids.



The Upper Eyelid Crease

One of the features of a youthful eye is a clear and natural upper eyelid crease. With ageing, the upper eyelid skin can begin to fold over the eyelid crease. This may make the eyes appear heavy, hooded or tired. In some people, this excess skin can even interfere with vision, by partially covering the eye.

Upper blepharoplasty, the process where the excess skin on the upper eyelid is carefully removed, and when necessary, a small pocket of fat is should be cut out. The goal is not to hollow out the upper eye socket and give a "done look", but rather restore a cleaner, and more refreshed upper eyelid contour.

An Open and Rested Appearance

Youthful eyes tend to look open, fresh and alert. However, this does not mean making the eyes look unnaturally wide. A good blepharoplasty should preserve your natural eye shape and facial expressions. The aim is to be subtle, reduce heaviness, improve definition and help you look more effortlessly rested - Not to become a different person.

The Lower Eyelid and Cheek Transition

The lower eyelid is just as important as the upper eyelid. A youthful lower eyelid usually blends smoothly into the cheek, whereas you may notice lower eyelid bags, tear trough hollows, shadowing or loose skin. These changes can make the eye look tired, even if the rest of the face looks youthful.

Beware of Festoons

Festoons, also known as malar bags or malar mounds, are areas of swelling or soft tissue laxity that sit over the upper cheek, just below the lower eyelid. They can make the eyes look persistently tired, puffy or swollen, and are often mistaken for ordinary lower eyelid bags. However, festoons are a separate concern and may not fully improve with standard lower blepharoplasty alone. This is why careful assessment is important: successful eyelid rejuvenation depends not only on removing excess skin or fat, but also on understanding the relationship between the eyelid, cheek support, skin quality and underlying anatomy.

Skin Quality around the Eyes

The skin around the eyes is thin and extremely delicate. Fine Lines, crepey lines, pigmentations and sun damage can ruin a youthful appearance. Surgery is the main way to improve structure and contour of the lower eyelids, but does not completely change the skin quality by itself.

For this reason, additional non-surgical treatments such as skincare, peels, and laser resurfacing can be a good adjunct to refreshing your look.

Brow Position Matters

Sometimes, the upper eyelid looks heavy because the brow has descended. If the brow sits low, it can push extra skin onto the upper eyelid. In these cases, an upper blepharoplasty alone may not address the problem fully. This is why the brow position should always be assessed before planning any eyelid surgery. Removing too much eyelid skin without considering the brow  can create an unnatural and tight appearance.


A tell-tale sign of brow descent is a progressive need to remove brow hair from the upper eyelid.
